[tcs-lc] For completeness - handling of ex authors

While we're raising hopefully minor issues - I mentioned this to
Roger (and I think we discussed it last week) and I know this is in
his 'do' list but I thought I'd get it out on to the list anyway

At the moment 'ex' authors are handled a bit clumsily - basically in
'Reckitt ex Coleman' the annotation suggests that the 'ex' should be
atomised in the same way as Messrs Reckitt and Coleman so you end up
with three agent names: Reckitt, Coleman and 'ex'. Everyone who's
seen this has said 'yuck'. Talking it over with Roger we thought this
could be handled with either an attribute or an extra element (there
seems to be a design backlash against attributes these days for some
reason)

This would have to be optional so if it went into a later version it
wouldn't break backwards compatibility but I think if it could make
it into the final voting version it would aid acceptance of the
schema
