Literature Inconsistency

chad's right - technically, the importing document can define any prefix it
wants. It only gets sticky when another schema imports the importing one and
defaultElementFormat gets set to "qualified" somewhere along the way. So
just to avoide problems we should at least be consistent within the release.
We dont have any control over how someone might define new schemas that
become added to "additionalMetadata"
