Yes, it's not sufficient because scenarios like comment#6 are still possible.

I think the best approach is to roll the report layout's LSID whenever the
report layout is changed. 

If you don't want to do it this way, but instead only on saves, then you need
to roll it before any save, which includes before the save to provenance when
the workflow is run.
