[Bug 558] - paragraph tag needs formatting structure

OK, so I responded to the feedback from Chad and Scott and did the 
following:

1) renamed paragraph back to para
2) made lists nestable
3) added subscript and superscript

Attached is an example instance document, and a picture of the current 
structure.

I think it is gettig pretty complex for a "simple" formatted text 
approach.  If we go much further up the complexity scale, I'd probably 
start arguing for wholesale import of the docbook schemas.

More feedback welcome, especially on the step/substep issue.  Thanks.
